CRITERIA FOR SECONDARY SPONSORSHIP

  • 1. Programs not offered at Otter Nelson River School.
  • 2. Exceptional (High Achievement Level) Gifted/Talented Students.
  • 3. Must be registered in three (3) courses minimum per semester.
  • 4. Must have a positive student record.
  • 5. Must have and maintain an acceptable average mark as set by the sending and receiving schools in compulsary courses.
  • 6. Must have positive behavior/attitude. eg. Drug/Alcohol Free, Zero Violence. Can be identified from previous school year.
  • 7. Students lacking 1 or 2 courses to complete high school and not offered at Otter Nelson River School.
  • 8. Students wishing to pursue studies in a special program leading to a particular career should apply for sponsorship prior to the beginning of the school year and must meet criteria
    numbered 2, 3, 4, 5, 6 stated above.
  • 9. Students participating in high calibre sports programs, host organizations to submit authorized documentation.

SPONSORSHIP APPLICATION INSTRUCTIONS:

PLEASE READ CAREFULLY AND FILL-OUT PROPERLY:
I. Top Portion - Student Identifier Code
- Please state your treaty nurnber.
- Include your position number.
- Please state your birth date - year, month, day - Date your application date.

2. Part A - Basic Student Information and previous education background (include program/course attempts)

3. Part B - Assistance Required - What kind of training?
When?
Where?
Please state your Career Goal, whether part time or full time and type of training For out of province, include prograrn information, tuition, books and travel costs on a separate sheet.

4. Part E - Debts (this is very important)

5. Part F - Please read the conditions for sponsorship and sign your application.

Note:
1. Please include your letter of acceptance.
2. Most recent transcript must be attached.
3. Applications will be processed when all documents are received.
4. Completed applications will be considered first.
5. Incomplete forms will be returned.
6. Applications are approved based on budget. 7. There is no appeal if funds are not available.
